Practice-Based Commissioning
PBC provides GP practices with the opportunity to reconfigure their services and to provide modernised clinical pathways.  They can now provide more services, including some previously located in the hospital sector.  Cost savings achieved in primary care against the PbR tariff can be significant and can be passed on to practices and PCTs.
 
Acton Shapiro has worked to transform primary care through effective commissioning, first supporting Primary Care Groups (PCGs) and now by working hand-in-hand with PCTs.  We can support GP practices with:
  • governance issues, where services are being delivered in an alternative setting
  • independent support for proposals when they are assessed by locality commissioning groups
  • legal structures (limited company, limited liability partnership, social enterprise, etc.)
  • organisational structures (medical/organisational leadership arrangements, etc.)
  • financial issues (including pension arrangements for staff and securing grants)
  • bid writing and procurement management
  • business planning.

The Southwark Provider Group (a GP-led organisation) in London, as they set up a social enterprise to provide services to their local commissioning group.
 
The “4Cs” Commissioning Group, for which the key rationale behind establishing the group (which is in North Bradford) was to develop effective joint working arrangements between GP Practices and other key stakeholders, in order to provide improved services and integrated care pathways which meet the health and social needs of the local population.
